Instead, you can submit your application in one of two ways:

1) Print out this form, fill it out, and a) mail it to Parkland Village, P.O. Box 8738, Albuquerque, NM 87198, or b) scan it and e-mail it to [email protected] OR

2) Copy this form, paste it into a file on your word processing application, fill it out on your computer, and then mail or e-mail it to us.
